How does SMAS lifting work with the Ulthera® System?
Ultherapy targets the Superficial Muscular Aponeurotic System (SMAS). The SMAS lies directly beneath the skin and consists of several layers.
The superficial layer is made up of collagen and elastin fibers, while the deeper layer is formed by facial muscles and their associated aponeurosis (a broad sheet of tendons).
Together, these structures form the framework responsible for skin elasticity and firmness. As we age, collagen and elastin stores deplete, fibers stretch and elongate, causing the skin to sag — resulting in the so-called gravitational ptosis. Wrinkles, double chin, under-eye bags, drooping eyebrows, and loss of jawline definition are common symptoms.
To eliminate these age-related gravitational changes, SMAS needs to be restored. This is the purpose of Ultherapy. When problematic areas are exposed to focused ultrasound radiation, the SMAS layers are heated precisely, causing soft tissue contraction. Ultrasound waves stimulate the production of collagen and elastin. After the procedure, the formation of new fibers continues for 3-4 months. The procedure can be repeated when signs of gravitational ptosis reappear. There are no contraindications for multiple treatments.

How the Ultrasound SMAS Lifting Procedure Works
- Before starting the treatment, the doctor performs makeup removal and face marking. Then, a specialized gel is applied to the skin to facilitate the smooth movement of the device’s handpiece.
- Ultrasound treatment: The doctor moves the handpiece along the marked areas. A pinpoint network is created in the layers beneath the skin, activating regenerative processes.
